Cancellation Terms

General Plan Cancellation Policy

Cancellation within 30 days: Patients may cancel their plan within 30 days of purchase for a full refund. After 30 days, refunds are not available, but the patient can cancel at any time.
Early Cancellation: If a patient cancels the plan before completing their annual term, the remaining balance will be due immediately, and no refund will be issued for unused months.
Cancellation Notice: Patients must provide at least 15 days’ notice before their next billing cycle for cancellation. If a cancellation notice is provided after the next billing cycle has started, the patient will be billed for that month, and the plan will be canceled the following month.

Periodontal Plan Cancellation Policy

The plan is non-refundable and non-transferable.
Services must be utilized within the 12-month period.
Cannot be combined with dental insurance or other promotions.
If the plan is canceled before the 12-month period is complete:
All utilized services will be charged at their full standard rates ($190 per visit, $395 for SRP, etc.).
Any remaining balance after deducting charges for utilized services will be refunded (for upfront payments) or adjusted from future monthly payments.
Cancellation must be requested in writing and approved by our office.
